Dermal Fillers for lip Enhancement – A Lip Reshaping Treatment

Why need dermal fillers?

As we age, the only problem we encounter is not just lines, there is also a loss of collagen, elastin and hyaluronic acid which gradually results in thinning of skin and lost in volume and skin elasticity. The change in the facial contours and volume is quite evident along with the changes in facial bones and the soft facial tissues. The graceful, attractive and lifted triangle of youth which comprise of high and full cheeks and a contoured small chin turns into an inverted triangle of youth giving a heavy bottom appearance to face as the cheeks become flat and jowls appear. Dermal fillers prove highly effective to tackle this facial condition.

What are dermal fillers?

Fillers Available at CosmeDocs: Teosyal | Restylane | Juvederm | Radiesse

Unlike Botox which relaxes the muscles, dermal fillers give volume and fill in the static lines, wrinkles, creases or areas of volume loss. They are used as ‘volumizers’ as they plump and lift cheeks, temples, give contoured jawline, fill in thin lips and sagging hands.

Types of Dermal Fillers

There are many different types of dermal fillers available but the most common and popular dermal fillers are hyaluronic acid fillers which are natural in substance. The results of these fillers are natural but temporary. The popular hyaluronic acid dermal fillers used at CosmeDocs include Teosyal, Restylane, Juvederm and Radiesse.

Hyaluronic acid helps in improving the hydration level of skin, promotes synthesis of collagen and normal skin functioning. It is vital for the structural unity of collagen matrix, giving structural support as well as providing nutrients.

CosmeDocs approach towards facial rejuvenation with dermal fillers is to carefully examine the facial features in accordance with the Leonardo da Vinci’s 3 facial zones (Upper Third, Middle Third & Lower Third). Our clinicians are bestowed with the artistic and creative abilities along with professional experience and expertise. This gives them a ‘double vision’ according to which firstly, they carefully examine and evaluate the areas and extent of facial volume loss which they manifest to their clients (most of them are even unaware of the gradual volume loss they are having over the years) and secondly, they are able to envision the results of dermal fillers treatment prior to its administration.

Role of Dermal Fillers in facial contouring and enhancement

Though dermal fillers treatment has been rated as the second most popular and commonly performed aesthetic enhancement treatment but it still raise scepticism and doubts in the minds of many people.

This is due to the unfavourable reports about dermal fillers treatment which lead to ‘pillow faces’ and ‘trout shaped pouts’. In addition to this, the role of dermal fillers has also been misperceived. The use of hyaluronic acid fillers as safe and effective treatment for wrinkles, lines and volume loss have been approved by the clinical studies reviewed by FDA

Basically, as the name of dermal fillers indicates they should fill up. However, they have much advanced role to play in addition to just filling up. However, they have much important and advanced role to play than just filling up wrinkles. In experienced and professional hands, dermal fillers can do subtle and refined facial sculpting and contouring.

Our highly experienced and professional doctors at CosmeDocs understand this very well and may recommend a treatment combination including dermal fillers, botox and other skin or cosmeceutical treatments to give improved and natural results. They use power of synergistic effects of different treatments in the similar manner as the artist use different colours to fill in their artistic master piece.

Forehead or pillow lines

Sleep or pillow lines which appear around the forehead are quite common. They can be caused due to various factors like sleeping position and the pillow type used. Due to the continuing sleeping style which affects the results of treatment, it is quite difficult to treat these lines.

However, we can soften and treat these lines by adjusting the sleeping position combined with collagen building treatments for skin including dermal filler treatment performed at the different areas of weakened skin layers and the cosmeceuticals which stimulates the production of collagen.

Cheek enhancement

Cheek enhancement also called malar augmentation is a highly desirable and advanced procedure which is performed by highly competent and experienced doctors at CosmeDocs clinics. As we age, the fuller and youthful cheek contour starts to flatten giving a sunken and tired appearance to face. With the appearance of malar grooves, the eyes can give a weary and drawn look. The facial volume is lost in the midface with the downward inclination of fat pads, reduction in elastin, collagen and hyaluronic acid.

Advanced dermal fillers treatment for cheek augmentation specifically targets flattened cheek areas or grooves. Our present highly advanced treatment technique makes use of cannulas with blunt and flexible ends. This causes a reduction in the sites of dermal filler injections to just one or two points of entry. The blunt end cannulas reduce the possibility of damage to the blood vessels and nerve to the minimum. This results in minimising the probability of bruising and the downtime of the treatment. You should choose cheek enhancement with dermal fillers with great caution. CosmeDocs have competent practitioners with vast experience and expertise in dermal fillers treatment which minimises the risk factors and ensure positive results.

Nosalablial Folds

These are the deep and diagonal folds that are present around the sides of the nose, extending towards the corners of the mouth or even further. With ageing, these furrows become deep and give a fatigued and harrowed appearance to the face. These deep grooves are caused by multiple reasons which include reduction in skin elasticity, tissue volume loss and descend of fat pads from the region of cheeks. Facial expression also plays a part over the period of time.

Nosalablial folds are treated with dermal fillers. The treatment procedure takes almost 10-20 minutes and there can be mild swelling, redness and bruising in the treated area.

Perioral Lines (Upper Lip Wrinkles / Smoker’s Lines) Lip Lines

Thin and wrinkled lips without contours and proper definition disturb many individuals as lips are a symbol of sensuality and youthfulness. Smoker’s line that appears vertically around the mouth contributes towards individual’s ageing appearance. These lines cause lipstick bleeding in women blurring up definition of lips.

There are many different treatments offered at CosmeDocs for treating these lines which are discussed by our experienced and professional cosmetic doctors with patients by adopting an individualized approach. Combination of Botox and dermal fillers treatment can be used to give fuller and plumper lips with well-defined outline and reduced wrinkles. Botox helps to relax the lip muscles which cause lines and wrinkles while dermal fillers are injected into the lines or into vermillion border to improve the definition of lips. Additional resurfacing treatments like peels and lasers can also be used to smoothen the lines.

Marionette Lines

The lines which extend downwards from the mouth’s corner towards the jaw are called marionette lines. These lines can give a look of despair and dissatisfaction to the facial appearance. Some people are genetically predisposed to the formation of these lines but there are also other factors influencing their formation including loss of soft tissues volume, bony changes, and reduction in skin elasticity, and active muscles in the lower face.

Dermal fillers treatment can greatly help in treating these lines by lifting the downturned mouth corners into a neutral position. In some cases, a combination treatment may be used in which botox is used for uplifting the mouth and dermal fillers is used for reducing furrows. For treating deep furrows, our expert doctors may recommend using dermal fillers over the time for treating the area which allows the accommodation of dermal fillers within the tissues.

Chin Augmentation & Jaw Line Contouring

Dermal fillers treatment can be used for improving the jawline and chin reshaping.

Flat and recessed chins can suppress the other facial features. Our skilled aesthetic doctors can successfully tackle this problem with the skilful use of dermal fillers. Before the procedure, interior and lateral view photographs of chin are taken. It is required that any dental or bony conditions should be excluded. Chin augmentation treatment using dermal fillers is appropriate only for the soft tissues modifications. Our expert and experienced aesthetic doctors will carefully examine any of your concerns and will recommend you the possibilities.

Chin enhancement usually requires strong structural support therefore it is necessary to use the filler that should give a firm filling effect. The recommended dermal filler for chin augmentation may include hyaluronic acid or calcium hydroxyapatite, like Radiesse®. Both of these dermal fillers offer strong scaffold.

While performing the procedure of chin augmentation, the jawline may also require examination and improvement. The pictures shown here have received a blend of chin augmentation and jawline improvement treatments to give more natural and attractive results.

Chin Mental Crease

It is a deep horizontal groove which is formed across the chin. This groove is formed due to various factors like thinning of dermal skin layer, loss of soft tissues, bony resorption, reduced skin elasticity and dental changes. The over activity of chin muscles may also lead towards chin dimpling along with prominent mental crease.

The best approach towards treating this condition is a combination of dermal fillers and Botox. The use of Botox relaxes the overactive chin muscle while dermal fillers help to soften the mental crease.

How many different types of dermal fillers are there?

There are many different kinds of dermal fillers but the most popular and widely used dermal fillers in UK are Hyaluronic acid based fillers. They are used because of their safety, effectiveness, versatility and easy administration. The different kinds of Hyaluronic acid fillers differ from each other in terms of their concentration, degree of cross-linkage and formulation. These factors affects the duration of their results and the extent of swelling after the treatment procedure. Most of the HA fillers now come with added lidocaine which helps in improving the comfort of patients and decrease the requirement of additional anaesthetic.

Radiesse® is very popular filler with long term effects. It provides great structural scaffold. It stimulates fibroblasts which help in synthesis of new collagen.

How will I look instantly after receiving the treatment?

The effect of fillers is evident after the procedure with mild redness, tenderness and swelling in the treated area. There is a risk of bruising which may follow immediately after the treatment. However, the bruising may become more evident in the days following the treatment but is a temporary condition.

What can I do to reduce bruising?

There are certain medications and supplements which thin the blood increasing the chances of bruising. Therefore, it is advised that 2 weeks before the treatment the patient should not take aspirin, Vitamin E, St John’s wort, gingkgo, evening primrose oil, garlic, feverfew and ginseng. It is also recommended that the patient should avoid taking alcohol and nonsteroidal anti-inflammatory medication like buprofen and diclofenac almost 2 weeks before the treatment.

2 days prior to the treatment Arnica tablets or gel may be advised to use and to continue their use after for 2-3 days after the treatment.

The use of cold compressions after the treatment helps in reducing swelling and cause constriction of blood vessels reducing further bruising.

If bruising has occurred, use warm compression for few days after the treatment as it will aid in speeding up the healing process.

Pineapple, papaya , topical and dietary vitamin C helps in quick healing as they help in breaking down proteins, build collagen and blood vessels.

If necessary, you can wear camouflage makeup, although it is highly recommended that it should be avoided on the day of treatment. Our experts can give you advice regarding the recommended make-up coverage.

What are the adverse complications that can occur?

A wide variety of dermal fillers treatments does not cause any complications. The tenderness, redness and swelling are temporary and vanish in few days. If bruising may occur, it almost takes 14 days to subside so it is important that you should plan the treatment in advance before any important event.

If any asymmetry ocuur, it can be settled by the use of additional dermal fillers. During and after the treatment a massage is done to improve any lumpiness or bumps. If the condition persists, the HA fillers can broken down by using the enzyme hyaluronidase.

Below are some of the very rare but serious complications:

Tissue Necrosis: In rare cases, obstruction in blood supply to the skin tissues can be caused by the injected fillers leading to necrosis. It may happen instantly after the treatment or after few hours. It is very important that the practitioner should be trained in handling such rare complications if they arise.

Granulomas: these nodules might appear after months or years of receiving the treatment. These are more commonly caused by permanent fillers. They are treatable with corticosteroid injections or excision.

Infections: as the process involves skin breaching, there exists a possibility of infections. To reduce the probability of risk of infections, the practitioner ensures that the area of treatment should be thoroughly cleaned and the equipment should be sterilised to minimise the risks of any kind of contamination. Any patient having a history of cold sores should be given warning about the risks involved of reactivating the infection. Prophylactic medication and treatment after the treatment procedure may be used to prevent the infections.

Scarring: Though it is quite rare, but scarring can occur if any infection occurs or if the patient has any medical history of hypertrophic or keloidal scarring. The medical history will be gathered prior to the treatment.

Is the dermal filler treatment more/ less painful as compared to Botox?

The dermal fillers treatment is quite tolerable with mild pain. The fillers come with anaesthetic which helps in minimising pain and if needed, topical anaesthetic cream can also be applied to the treatment area. As the dermal filler treatment procedure make use of varying injection techniques, it normally takes slightly more time as compared to Botox due to which it may seem to be painful.

How much dermal fillers are usually injected?

The Hylauronic acid fillers are genrally available in 0.5 and 1ml syringes. The quantity of fillers to be injected depends largely on the area of treatment and type of filler used.

Radiesse® or Calcium Hydroxylapatite comes in 0.3, 0.8 and 1.5ml syringes and is usually used for semi-permanent structural filling. The volume to be used for the treatment is decided after examining the treatment area.

Will the filler become lumpy?

After the treatment procedure, our expert practitioner will gently massage the treated area which will smooth any evident lumps. It may be recommended to do a gentle massage at home with the finger tips if any lumps are felt for 48 hours. Generally, the dermal filler takes its place within 3-4 weeks after the treatment and does not feel unnatural. If still you feel the bump, you should consult your practitioner who will correct the lump with massage or will breakdown the product.

Can I be allergic to the dermal filler?

Though it is very rare but there are possibilities. This may result in mild allergy which could cause prolonged swelling or a severe hypersensitivity with swelling, redness, bumpiness and a rash. Antihistamines and steroids are given to counter the allergic reactions by the practitioner.

Can there be any movement of filler after its injection?

If you aggressively massage the treated area, there is a possibility that the filler moves. Therefore, it is advised not to touch the treated area unless you feel any lumps or bumps on which you can apply gentle pressure.

Can I see the injected filler?

It is quite rare that you see the filler after injection. HA fillers are colourless and does not show if injected in a correct way. If filler is not injected properly in a superficial layer then it may appear as a bluish skin discolouration. This condition is manageable with mild compression or in some cases an injection may be used to break down the product.

Will I feel any difference in lips/ skin after the treatment?

After the swelling resides, it almost take 2-3 weeks for the filler to settle at its place and will feel natural just like the surrounding skin/lips.

Will other people be able to identify the difference?

Generally, people appear better without any obvious or unnatural changes in their appearance. A good aesthetic doctor will replace the volume loss giving an overall rejuvenating effect to the facial profile. Initially, there may be swelling in the treated area so it may be seen by the friends or family.

How often do I need to have fillers treatment?

This varies from individual to individual. Majority of fillers may remain for up to 6-12 months however as fillers also stimulate the production of collagen and gives plump to the skin, it may not be necessary to undergo repeat treatment after this specified time period. Unluckily, for some individuals, the product breakdown may occur more rapidly as compared to others. The best advice you can get is at consultation or review appointments.

Is it fine to have a new brand of dermal filler injected after having some other brand in the previous treatment session?

Generally, there is no issue if there had been no complications involved in previous treatment. You need to tell your practitioner when you have the previous treatment, which product was used and why you want to change the product.
