Some people especially women possess constant downward mouth corners. The Furrows or marionette lines appear from the corners of the mouth (oral commissures) which gives an expression of sadness or melancholy to the face. This downturned smile results from the downward pull of the depressor muscle from the corners of the mouth.

Treatment options: The muscle is required to be identified on grimacing for the Botox treatment. Then small doses of Botox are injected into the origin of the muscle on both sides of the jawline. Botox weakens the muscle which pulls the corners downward and allows the levator muscle of the mouth to pull the corners in the upward direction.
